FORM NO.2 (RULE -5) APPLICATION FOR THE GRANT OF A LICENCE TO A MONEY LENDER IN THE OFFICE OF THE REGISTRAR OF MONEY LENDERS ANDAMAN AND NICOBAR ISLANDS The applicant (undersigned) hereby applied for grant of a licence under section 5 of the Andaman and Nicobar Islands Money Lenders Regulation, 1956 (Regulation No.2 of 1953?). In case where the applicant is a company or an incorporated body the name of the Director and Manager or Principal Officer managing the company or the incorporated body with father's or husband's name, surname, residence and address in full : 4. In case where the applicant is a firm or any other kind of an un-incorporated body the name of all the persons constituting the form or forming the un-incorporated body, with father's or husband's name, surname, residence and address in full : 5. Name in which the applicant carriers on or intends to carry on his money lending business. : 6. Particulars of the are with in which the applicant has his business or money lending on the date of the application. : 7. Name of the person responsible or proposed to be responsible for the management of the applicant's money lending business with his father's or husband's name, surname, residence and address in full. : 8. Location of the applicant's place or principal place of business with full particulars thereof and the name, father's name or husband's name, surname and address of the person in charge. : 9. Whether the person signing the application of an of the adult coparceners of an un-divided Hindu Family, or any Director, Manager or Principal Officer of the company of the in-corporated body or any member of the un-incorporated body on behalf of which the application is made, as the case maybe carried on the business of money lending in the Andaman and Nicobar Islands in the years ending on the 31st day of December immediately preceding the date of the application either individually, or in partnership or jointly with any other coparcener or any other person and whether in the name or any other name. : 10. Is the applicant or another persons on behalf of whom the application has been made carrying on or intends to carry on the business of money lending in any other place in the Andaman and Nicobar Islands?
