Health Assistant 100 marks obtained in the S.S.L.C. or its equivalent and 100 marks obtained in the Diploma in Multipurpose Health Worker (Male) or Diploma in S`anitation or Diploma in Health Inspector Course/Health Assistant / Diploma in Public Health and Sanitation awarded by recognized University / Institution. 3. A deduction of 5% of marks will be made for every additional attempt either to pass the examinations or to improve the results, while computing the marks for educational and technical qualifications in respect of all the posts. 1.5 marks will be awarded for every completed year of seniority from the date of registration of requisite technical qualification acquired in the Employment Exchange to the date of notification of vacancies will be added subject to a maximum of 15 marks in respect of posts at Serial No. from 01 to 03 only. 2. The No. of vacancies and reservations for various categories indicated for the above mentioned posts are only provisional and subject to variation depending upon the requirement. Ex-servicemen 10% Group 'C' posts iv. Meritorious Sportsperson Not exceeding 5% No specific percentage prescribed. The overall reservations will not exceed 50% of the total no. of vacancies. v. Blind, Deaf-mute and 3% Three per cent @ 1% for each of the three categories. However, Orthopaedically the reservation of 3% shall be equally distributed between handicapped persons persons with the categories of disabilities for whom the post has been identified as suitable. 3. Eligibility: Age: Between 18 - 32* years as on 14/05/2010 which is the last date for receipt of filled in application. (*The upper age-limit for recruitment by the method of Direct Recruitment as per Rules (Upper Age-limit for Direct Recruitment) Rules (1998) increased by two years w.e.f. 01.04.1999.) (4) Sub-Divisional Officer of the area where the candidate and/or his family normally resides. (ii) Most Backward Class / Other Backward Class For posts filled by direct recruitment 3 years over and above the prescribed age-limit. Note :SC / MBC / OBC candidates who are selected on the same standard as applied to general candidates shall not be adjusted reserved vacancies. In other words, when a relaxed standard is applied in selective an SC/MBC/OBC candidates, for example in the age-limit, experience, qualification, extended zone of consideration larger than what is provided for general category candidates, etc., the SC/MBC/OBC candidate will be counted against reserved vacancies. Such candidates would be deemed as unavailable for consideration against unreserved vacancies. (iii) Ex-servicemen and demobilized personnel of Armed Forces For posts filled through Employment Exchange / Department of Sainik Welfare, Puducherry Service rendered in Army, Navy and Air Force plus 3 years Note : Reservation benefit can be extended only in the first Civil Employment. Once he reemployed in Civil Employment, he ceases to be an Ex-serviceman and not entitled to the benefit of reservation for other posts subsequently. However, the benefit of age relaxation is admissible to such re-employed persons for securing another employment in a higher grade or cadre in Group 'C' posts. An Ex-serviceman selected under the reservation provided for them should be placed in the appropriate category, viz., GEN/SC/MBC/OBC for the category to which he applied for. (iv) Meritorious Sportsperson All Groups of Civil posts Up to 5 years (10 years for SC candidates) MERITORIOUS SPORTSMEN : (i) Persons who have represented a State or the country in the National or International competition in any of the games / sports listed below. (ii) Their University in the Inter-University Tournaments conducted by the Inter-University Sports Board in any of these games. (iii) The State Schools Teams in the National Sports / Games conducted by the All India School Games Federation in any of these games / sports. (iv) Persons who have been awarded National Awards in Physical Education under National Physical Efficiency Drive. List of Games / Sports - Archery, Athletics (including Track and Field events), Atya-Patya, Badminton, Ball-Badminton, Basketball, Billiards and Snooker, Boxing, Bridge, Carrom, Chess, Cricket, Equestrian Sports, Football, Golf, Gymnastics (including Body-Building), Handball, Hockey, Judo, Kabaddi, Karate-DO, Kayaking and Canoeing, Polo, Kho-Kho, Powerlifting, Rifle Shooting, Roller Skating, Rowing, Softball, Squash, Swimming, Table Tennis, Taekwondo, Tennikoit, Tennis, Volleyball, Weight-lifting, Wrestling and Yatching. Note : Certificate to a Meritorious Sportsman for Employment to Group 'C' and 'D' services under the Government shall be in the prescribed form issued by the Competent Authority awarding certificate will only be accepted. (v) Blind, Deaf-mute and Orthopaedically handicapped persons For Group 'C' and 'D' posts filled through Employment Exchange Ten years (15 years for SC) and 13 years for OBC. Persons suffering from not less than 40% of the relevant disability shall alone be eligible for the benefit of reservation. Note : Percentage of reservation : Three per cent @ 1% for each of the three categories. However, the reservation of 3% shall be equally distributed between persons with the categories of disabilities for whom the post has been identified as suitable. (vi)Departmental For appointment to Group 'C' and 'D' by Up to 40 years of age (45 years for candidates with 3 years direct recruitment which are in the same SC) continuous service line or allied cadres where a relationship can be established that the services rendered by them in the department will be useful for efficient discharge of duties in the post for which selection is made from time to time GENERAL INSTRUCTIONS 1. The applicant should necessarily submit attested copies of the following certificates: a. Birth certificate / Transfer Certificate or Board / University Certificate indicating Date of Birth for age proof. b. S.S.L.C./Matriculation/Higher Secondary or its equivalent Mark Sheets wherever it is required or mentioned so under col.5. Education / Technical qualification prescribed in the notification / advertisement of the post concerned. c. Necessary Degree/Diploma or Certificate relevant to the above education/technical qualifications with all Marks Sheets (year-wise / semester-wise) including attempts made, if any. d. The educational/technical regular course prescribed should have been completed prior to the last date for receipt of filled in application. e. Necessary registration certificate as a Midwife in anyone of the State Nursing & Midwifery Council in India for the post of ANM / Maternity Assistant and Registration Certificate from the Pharmacy council duly renewed as on date. Permanent Registration Certificate for Nurse and Midwifery / Psychiatry Nurse from any State Nursing & Midwifery Council in India, wherever it is required (Note : In the event of selection to the post, offer of appointment will be given only on production of permanent Registration Certificates). f. Resident / Nativity Certificate : The candidates who are natives of the Union Territory of Puducherry by continuous residence in the Union Territory of Puducherry for the last 5 years immediately preceding the date of notification only are eligible to apply for the posts. (Note : Certificate issued on or after 01.01.2010 will only be accepted.) g. Most Backward Class / Other Backward Class / Scheduled Caste Certificate for employment purpose obtained from the Competent Authority of the Department of Revenue and Disaster Management, Puducherry/Karaikal/Mahe/Yanam not below the rank of Executive Magistrate / Tahsildar / Deputy Tahsildar as the case may be issued on or after 01/01/2010. h. Certificate obtained from the Competent Authority of the Department of Revenue and Disaster Management, Puducherry/Karaikal/Mahe/Yanam in the prescribed form alone will be accepted. Old and combined certificates will not be accepted. i. If migrant certificates are submitted in respect of MBC / OBC / SC their applications will be considered against Un-reserved category. j. Candidates who have to apply against the horizontal reservation provided for Exservicemen, Meritorious Sports person and Physically Handicapped person, should necessarily submit the relevant certificates issued by the Department of Sainik Welfare, Puducherry, Department of Sports and Youth, Puducherry and Department of Health & Family Welfare Services / Medical Authorities, Puducherry respectively. Certificates received in any other form will not be accepted. k. Employment Exchange Identity Card duly renewed as on date, bearing NCO code Nos. for the qualifications acquired by the candidate. l. Original Educational / Technical Mark Sheets / Certificates should not be attached with the application 2. The applications which are not in complete shape, or without enclosing the requisite certificates / mark sheets etc. as indicated above, will be summarily rejected and further correspondence will not be entertained for any reasons therefor. The candidate shall ensure himself / herself that all requisite copy of certificates are attached with his / her application on the date of submission of application and the Department will not be responsible for any lapse in this regard. Copy of certificates left out if any proposed to be enclosed after the date of submission of application by the candidate will not be accepted and requisition in this regard will not be entertained on any account. No intimation will be given for receipt of late / incorrect / ineligible applications and also the result of recruitment would not be intimated on any account to the non-selected candidates. Only the selected candidates will be issued offer of appointment. No correspondence or interim enquires from the candidates will be entertained. 3. Applications received after the prescribed date and time will not be considered. Application, which is incomplete or wrongly filled in will also not be considered and such application will not be returned. 4. Candidate should submit only one application for one post (Instructions / General Instruction to candidates attached with the application form should not be enclosed). If the candidate desires to apply for more than one post, separate applications should be submitted for each post along with requisite attested copies of certificates / mark sheets etc. Common application for two or more posts will not at all be accepted. 5. Government servants working in the State/Central Government /Autonomous Bodies and Public Sector Undertakings should submit their application through their Employer only. Advance copy should not be sent and it will not be considered. 6. The Director, Directorate of Health & Family Welfare Services, Puducherry reserves the right to enhance / reduce the no. of vacancies and also to cancel the recruitment process, if necessity arises. The decision of the Government shall be final regarding the eligibility of candidates or otherwise. NOTE : AS PER HIGH COURT ORDER UNDER WRIT PETITION NO.28572 OF 2004, 47866/2006 & M.P.No.1 of 2006, 14203, 9097 and 9098 OF 2007, CANDIDATES WHO HAVE ALREADY APPLIED TO THE POSTS OF ANM / MATERNITY ASSISTANT, PHARMACIST (ALLOPATHY) AND HEALTH ASSISTANT PURSUANT TO THIS DIRECTORATE'S ADVERTISEMENT NO.A.12020/4/2004/E4/DHFWS DATED 27.05.2004 NEED NOT APPLY AGAIN IN RESPONSE TO THE PRESENT NOTIFICATION. HOWEVER, THE CANDIDATES WHO HAVE ALREADY APPLIED TO THE POST OF ANM / MATERNITY ASSISTANT PURSUANT TO THE EARLIER NOTIFICATION DATED 27.05.2004, ELIGIBLE IN ALL RESPECTS AS ON 14.06.2004, HAVE NOW TO PRODUCE A COPY OF MATRICULATION OR ITS EQUIVALENT MARK SHEETS. COPY OF THE EMPLOYMENT REGISTRATION CARD WITH THE SPECIFIC ENTRY OF TECHNICAL QUALIFICATION THEREIN WITH N.C.O. CODE AND DATE OF REGISTRATION FOR THE POST OF ANM / MATERNITY ASSISTANT, HEALTH ASSISTANT AND PHARMACIST (ALLOPATHY) SHOULD BE FURNISHED FOR COMPUTATION OF 1.5 MARKS. (See Note 1 below Instructions to Candidates) CANDIDATES WHO HAVE ALREADY APPLIED (WHO ARE AGE BARRED NOW) TO THE POSTS OF ANM / MATERNITY ASSISTANT, PHARMACIST (ALLOPATHY) AND HEALTH ASSISTANT PURSUANT TO THE EARLIER NOTIFICATION DATED 27.05.2004, WHOSE AGE AS ON 14.06.2004 WILL ALONE BE TAKEN INTO CONSIDERATION, IF THEY ARE OTHERWISE ELIGIBLE.
